Cornerstone Health Care, Inc. provides skilled home nursing up to 24 hours daily, 7 days a week to medically fragile and medically complex newborns, infants, children and adolescents to 21 years of age.

We understand the challenges of caring for a medically complex child who requires numerous medical services and products. Our experienced case managers, along with our bedside RNs and LPNs, work closely with parent and physician to ensure a thorough, expertly coordinated plan of care is implemented. We provide full parent education on all clinical services and we are able to provide care in private homes, school, medical foster homes, and our own medical group homes.

Complex Medical Conditions:
Medically fragile infants and children receive a wide range of clinical interventions including apnea monitoring, oxygen administration, tracheostomy care, ventilator management, g-tube feedings, medication and formula administration, and much more. Medically fragile conditions include, but are not limited to the following:
